Partnerships
In 2002, Hospice formed a partnership with Matthew House, a 2-bed home for the terminally ill located at 43 Metcalf Drive in Auburn. In August of 2002, Matthew House admitted their first resident. Hospice has been providing the professional medical management at this wonderful home, working hand-in-hand with Matthew House. For more information, contact Wendy Young at Matthew House at 315.252.2052 or Terry Kline at Hospice of the Finger Lakes at 315.255.2733.
In May 2004, Hospice and Auburn Memorial Hospital began working more closely together to provide quality end-of-life care to patients in need. This new inpatient service in known as the Comfort Care Program.
Under the Comfort Care Program, Hospice is able to admit patients who are hospitalized and qualify for Hospice inpatient services. These patients may be quite ill and the family unable to manage care at home; or they may be awaiting placement at an area health care facility, e.g., nursing home or Matthew House.
Patients who qualify for the Comfort Care Program will receive the same loving care that Hospice provides in a home setting. With thanks to the Hospital Auxiliary, items have been purchased for Comfort Care Carts (flowers, quilts, books and a comfortable chair), which will make it possible to create a homelike atmosphere in a hospital room. Specialized patient care will be provided by Hospice including nursing support, social work, pastoral care, volunteers and bereavement care for the family.
For more information about the Comfort Care Program, contact Social Work Services at Auburn Memorial Hospital at 315.255.7387 or Terry Kline at Hospice at 315.255.2733.
Early in 2003, Hospice formed another partnership with Cayuga County Nursing Home. In mid-May, Hospice admitted our first nursing home resident to Hospice care. We have provided nursing support, guidance with pain management, and volunteers for companionship. This is a new service for our community, however not for Hospice in other areas.
